Personalized (Custom) Keepsake Plaque Purchases

  • Use a separate order form for each personalized custom plaque purchase.

  • Plaque price is $49.99 each.  Add 5% sales tax for Maryland residents ($2.50, for total of $52.49).

  • Shipping within the U.S. is via USPS Priority Mail, FOB Shipping Point.

  • $5.50 shipping and packaging per plaque.

  • All personalized custom plaques are non-returnable and non-refundable.

  • Requests for changes after order will incur additional charges.

  • Payment must be received and verified before we begin work on your personalized plaque.

If paying via PayPal, be sure to go to Order Form to fill out baby information, etc.  Alternatively, you may email the information (see below) to  Orders@StorkHeadquarters.com.  Our online order form is under construction at this time.

Under normal circumstances, plaques will be shipped within 7 to 10 days after your payment has been verified.  However, during high volume times, your plaque may be shipped later.  We will notify you via email or fax of our approximate shipping date.

Stork rentals are available in select areas in Harford County, Maryland and Baltimore County, Maryland.  Please call us at 410-838-7737 to check availability, pricing, payment arrangements, and to place an order. Payment terms are COD. We accept credit card payments via PayPal invoice.
